• Appointments must be scheduled in advance by phone or through our online request form. Submitting an online request does not confirm an appointment. Our team will contact you to finalize the date and time.

• Please provide at least 24 hours notice to cancel or reschedule. Late cancellations and missed appointments may be subject to a fee.

• Repeated missed appointments may affect your ability to schedule future visits.

4. Payment and Insurance

• Payment is due at the time of service unless other arrangements have been made in advance.

• We participate with many major dental plans and will file claims as a courtesy. Verification of benefits is an estimate only and is not a guarantee of payment by your carrier.

• You remain responsible for deductibles, copayments, non covered services, and any balance your insurance does not pay.

• Balances not resolved within the terms provided on your statement may be subject to collection.

Any insurance checking tool or benefits estimate offered on our website is provided for convenience. Final coverage determinations are made by your insurance carrier.

5. Patient Responsibilities

7. Emergencies

Our website and text messaging service are not monitored around the clock. If you are experiencing a dental emergency, call our office during business hours. For a medical emergency, severe swelling, uncontrolled bleeding, or difficulty breathing or swallowing, call 911 or go to the nearest emergency room immediately.
